A Shared Living Provider (SLP), is a community-based living option for individuals with an intellectual or developmental disability. A Shared Living provider supports a person who comes to live in their home with daily living skills, providing transportation to medical appointments, work, and getting out into the community. A Shared Living home becomes home for the person served.
The individual pays monthly room and board to the Shared Living provider. The subcontractor is paid through Goshen for support services covered by state funds allocated for residential services.
How can someone become a Shared Living provider?
Goshen looks for individuals that are caring, supportive and who have an extra room available in their home. It is preferred that the Shared Living provider have some experience with individuals with intellectual or developmental disabilities, but it is not a requirement. Applicants with backgrounds in human services, medical services, education, or counseling usually make great Shared Living providers.
Our goal is to create the best match possible between the subcontractor and the person desiring supports. Contractors must agree to a 12-month contract with the individual. However, it is hoped that the relationship will last much longer. Goshen ensures that the Shared Living provider meets all regulatory requirements, and provides training to provide quality services and supports.
